New Patient Registration
To become a patient at any of our community health centers, you can call the health center directly or contact our Health Access Line at 802-692-7240. They can help you find a location and provider who will be a good fit for your needs and help you complete the patient registration process.
As a new patient, you’ll need to complete some patient registration forms before your first visit, including a consent form to have your medical records released to us from your previous provider.
