We began integrating behavioral health with our primary care services in 2009. Our behavioral health staff work in close partnership with you and your medical care provider as a team led by your primary care provider. Together, we address the physical, behavioral and emotional aspects of your overall health.
Our Program Includes:
– In-office consultations
– Screening and assessment
– Brief behavioral health interventions
– Stress management
– Psycho-educational skills training and development
– Referral services to specialty mental health practitioners
Who is Eligible to Receive These Services? Behavioral Health services are available to all patients within Springfield Medical Care Systems as part of their overall health care.
How Can Interventions Help? Short-term assistance (1-4 meetings) are an important step to addressing behavioral health concerns. These can be at the time of a primary care visit or during separate scheduled visits. Our behavioral health consultant typically does not provide ongoing psychotherapy services. They connect patients with additional resources and support within their communities or facilitate referrals for more in-depth, specialized services when needed.
