As a National Committee for Quality Assurance (NCQA) Medical Home, we strive to exceed the standards around access and continuity, population and high-risk management, self-management and community care resources. We carefully coordinate any specialty or hospital care you may need so you receive the right care, at the right place, at the right time.
Our on-site services include: primary and preventive care, pediatrics, outpatient mental health and addiction counseling, minor skin surgeries, obstetrics & gynecology and women’s health services, orthopaedics and sports medicine, nutrition counseling, and planned diabetic visits. Lab and x-ray services are available on site. Referrals to medical specialists, including dental and vision care, will be coordinated by your primary care team to provide you high-quality, comprehensive care that meets your personal needs.
