Same Day Appointments

Establishing a primary care “medical home” is recommended to manage your health in the best possible way. For unexpected (but non-emergency) healthcare needs, same-day appointments in our primary care offices may be available, and we encourage you to call to ask whether you can be seen on a “same-day” basis. If no appointments are available, walk-in care (with no appointment necessary) is offered in our Bellows Falls, Londonderry, Ludlow and Springfield, VT locations, and our Charlestown, NH location.

After-hours Care

For after-hours care, please contact the SMCS Community Health Center where you receive your primary care.

Please call 9-1-1 if you need emergency transportation.

Late Arrivals

If you are late arriving for your appointment we will make every effort to serve your needs, as time allows and being respectful of previously-scheduled appointments. You may be given an option to wait or be invited to reschedule to a time more convenient for you.

Health Information Management

If you are looking to obtain, transfer or share your medical records, please complete a Medical Records Release of Information form and contact our Health Information Department at 802-885-7307.
