If you have an unexpected illness, same day appointments are available, as well as walk-in services. Walk-in is available seven days a week, with no appointment necessary.

We are here to work with you as your personal care team, while striving to exceed the standards around access and continuity, population and high-risk management, self-management and community care resources. Referrals to medical specialists, dental and vision care can be coordinated during consultation between you and your primary care provider. We will make every effort to meet your needs in a timely manner.

Who is Eligible to Receive These Services? Care will be provided to all, regardless of ability to pay. Financial assistance is available to those who are income-eligible and fees are charged in accordance with the sliding fee discount program based on family size and income. Our health center does not discriminate based on gender, skin color, race, ethnicity, physical disability, religion, or sexual orientation of anyone seeking care.

SMCS is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

Medical staff is on call. In the event of an urgent issue after hours, please call 802-228-8867.

In the event of an emergency, please report to the nearest emergency department or call 9-1-1 for emergency transport.
